POSITION SUMMARY
In conjunction with the Manager, the Home Care Supervisor is responsible for assuring the delivery of timely and effective care to patients in the home care setting. The Supervisor has responsibilities in the areas of planning, organizing, budgeting, supervising, evaluating, staffing, scheduling, training and educating associates assigned to home care. The Home Care Supervisor position includes oversight of an interdisciplinary care team, patient and associate education, orientation, service line operations, budget, quality improvement, outcomes, and accreditation and regulatory compliance.
PERSONAL SPECIFICATIONS
A. Education
Writes, reads, comprehends and speaks fluent English.
Graduate of an accredited school of professional nursing, therapy, or other related program.
Bachelor degree in clinical or health care management preferred.
B. Qualifications/Experience
Licensed or certified as required by discipline to practice in the State of Iowa.
Clinical skill competence within own discipline.
Competence in mentoring, teaching and basic management skills.
Previous leadership experience and minimum of three years clinical experience.
Applies well-developed human relation skills in individual and group interactions.
C. Special Skills/Knowledge
Experience in an interdisciplinary home care model.
Comprehensive knowledge of nursing theory and practice and an expanded knowledge of current clinical and specialized patient care concepts related to own professional clinical service area.
Competence in computer word processing, spreadsheet and internet skills.
D. Mental and Cognitive Traits Required
Good verbal and written communication skills.
Able to analyze written, visual and other sensory stimuli.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The associate is aware of hospital and department compliance for federally-funded healthcare programs (e.g., Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of department or hospital administrative staff.
Purposefully conducts all aspects of the job in an ethical manner in support of the hospitals commitment to ethical behavior in all areas of personal and professional activity.
Successfully completes hospital-wide orientation, annual hospital competencies and department/unit-specific competencies.

Program Planning and Development
Collaborates with management, physicians and others to facilitate planning, implementation and evaluation of strategic plan and other initiatives including recommending annual goals and objectives for assigned area.
Assists with planning and implementation of written goals and objectives.
Effectively communicates hospital and service line mission, vision and plans.
Facilitates the organization and coordination of the delivery of services and care provided through assigned area.
Assists with development of annual FTE, operations, and capital expense budgets for assigned area.
Resource Management
Assists with management of operations for the home care agency. This includes measurement of the department's effectiveness using patient satisfaction ratings, staff satisfaction ratings, and financial data.
Investigates variances and initiates corrective action or justifies non-adherence to quality, financial and patient satisfaction outcome goals.
Maintains appropriates staffing levels to meet patient care and service needs.
Monitors and adjusts productivity as it relates to guidelines developed within each discipline.
Patient Care
Models patient outcome focused care and customer centered philosophy in practice.
Assures provision of patient care to meet patient needs through delivery of service in a timely manner and according to the goals and scopes of services for assigned area.
Assists Manager with ensuring development and implementation of standards of care for the patient population.
Assists with assuring compliance with standards of The Joint Commission, DIA, Iowa Board of Nursing Examiners and other applicable regulatory bodies.

Assists with home care and post-acute division performance improvement.
Maintains appropriate quality assurance and outcome programs.

Ensures assigned area is adequately staffed.
Evaluates daily staffing needs to determine schedule changes and staff reallocation. Collaborates with all relevant acute and outpatient areas to determine schedules and staff reallocation.
Anticipates staffing and scheduling needs, overtime usage and vacation replacement needs.
Provides an orientation program for new department associates and assigned students based on principles of adult learning.
Assists with in-services and continuing education programming to meet identified staff development needs.
Coordinates weekend and after hours scheduling and report to assure coverage for clients needs.
Relationships
Models collaborative behaviors that facilitate achievement of department and program goals.
Ensures that proper communications and plans are coordinated with other hospital departments.
Participates in recruitment, interviewing, selection, promotion, transfer, status change and L.O.A. decisions.
