Hospice Aide

• Assist the patient to maintain good personal hygiene by assisting with bathing; care of skin, hair nails and mouth; and changing of bed linens.
• Help patients get in and out of bed and assist them to ambulate as appropriate.
• Assist patients in the use of durable medical equipment.
• Report to the RN Case Manager any change in the patient's medical, nursing, rehabilitative and social needs as the changes relate to the plan of care and quality assessment and improvement activities.
• Plan and prepare nutritious meals in accordance with dietary restrictions and assist in feeding patients when necessary.
• Maintain a healthy, safe and clean environment for the patient.
• Perform homemaking duties as assigned.
• Assist patients with prescribed exercises, when required.
• Maintain up-to-update clinical knowledge by attending at least twelve (12) in-service hours per year.
• Attend interdisciplinary, staff and Aide meetings as required.
• Maintain and complete appropriate documentation records as required by Hospice policy, state and federal
• requirements. If documentation is not prepared and turned in according to these guidelines I understand that
• my compensation for this work will be withheld until I have completed to the required documentation.
• Adhere to the practices of confidentiality regarding patients, families, staff and the Organization.
• Follow Aide Care Plan. Document all activities daily.
• Submit paperwork to the office on a weekly basis.
• Ensure that Grace Healthcare Services' best practices are incorporated into all job activities/responsibilities.
•  Perform other duties as requested.
