Registered Nurse-Home Health

The Cardiac Connection is seeking an experienced critical care or progressive care nurse to provide home health care for patients with complex medical illnesses.

The home health nurse provides education, treatment, and emotional support for patients with a variety of medical illnesses and of any age group.  The nurse is able to work independently and must have strong assessment and critical thinking skills.  Good communication skills are a must as the nurse acts as a liaison between patients, their families, and other healthcare professionals to ensure safe, efficient, and quality health care services.  The nurse must be comfortable with electronic documentation using a tablet and have good computer skills.

Registered Nurse Responsibilities:

• Administer nursing care to ill, injured, or disabled patients
  • Wound care: Including wound vacs and ostomy care
  • Lab draws: venipuncture and central line
  • Management of patients with: Enteral tube feeds, chest tubes, home oxygen, peritoneal dialysis, tracheostomy
  • Assessment and management of complex and/or acute care patients including Advanced Heart Failure patients with a home infusion or a Ventricular Assist Device
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members
• Attendance to weekly case conferences
• Participation in educational activities for professional development

Registered Nurse Qualifications:

• 3 years of previous critical care or med surge experience in nursing
• BSN preferred
• Active Registered Nurse License in the state of VA
• Certifications: BLS required, ACLS preferred, Specialty Certification preferred
• Familiarity with computerized medical documentation and equipment 
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
