Essential Functions and Responsibilities:

Assess inpatients on the service, review diagnostic studies, and summarize the needs of the patient, health care providers and family.

Demonstrates competence in the skills necessary to carry out assigned job duties.

Maintain regular communication with physician(s) regarding status changes and urgent issues related to a patients care.

Act as a liaison between the physician and the floor nurses.

Evaluate new patients, provide feedback to interdisciplinary team and communicate findings/recommendations to physician(s) timely.

Facilitate the care coordination across service lines.

Provide education for patients and their family specific to their diagnosis, care plan, and ways to access community resources when necessary. Coordinate family conferences with physician as appropriate.

Responsible for coordinating the patient care team in providing quality, cost- effective patient care facilitating use of appropriate clinical order sets.

Monitor the patient care process and collect data to evaluate and improve care. Collaborate with physicians and other health care workers.

Work with the health care team to coordinate discharge planning early in the admission. This may include referrals after discharge, patient placement in skilled or assisted living settings, diagnostic studies, and home health care as determined necessary.

Be aware of what is happening in clinic/department and the organization by attending clinic/department meetings, reading emails and regularly checking information on the organizations intranet site.

Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines.

Maintain regular and consistent attendance at work.

Maintain compliance with Personnel policies and procedures.

Behave in a manner consistent with all Compliance and HIPAA policies and procedures.

Monitor environmental conditions in order to secure protected health information.

Demonstrate initiative to improve quality and customer service by striving to exceed customer expectations.

Balance team and individual responsibilities; be open and objective to others views; give and welcome feedback; contribute to positive team goals; and put the success of the team above own interests.

Perform other duties as requested by Hospital Program Manager or Clinic Administrator to facilitate the smooth and effective operations of the hospitalist service.

Supervision Received:

General supervision is received from the Hospital Program Manager or Clinic Administrator.

Qualifications:

Graduate of an accredited program for Registered Nurses.

Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse.

Meets educational/competency requirements per policy.

Ability to use the computer effectively in order to navigate several electronic programs related to patient care.

Mandatory Reporter certification.

CPR certification.

Strong interpersonal skills.

Ability to work as a team member.

Ability to understand and apply guidelines, policies and procedures.

3-5 years of nursing experience preferred.

Bachelors education in health related field preferred.

Strong verbal and written communication skills.
