Job Summary:

The Medication Access Coordinator is responsible to drive and increase prescription capture rates for CHI Health Specialty Pharmacy (SP) by interfacing with provider staff, patients and CHI Health SP operations management. Represent CHI Health SP and operate as the relationship and Medication Access Coordinator between the provider staff, patient, pharmacy, billing, insurance provider, patient communities, social services and other involved parties to drive excellent customer service delivery and profitability of CHI Health SP. Problem-solve fulfillment service and insurance concerns and partner with all involved parties to ensure the patient receives the best possible care experience. Develop and implement a comprehensive strategy for outreach efforts, allowing for regular improvement assessments and modifications. Responsible for managing the performance and effectiveness of outreach efforts to assure measurable business results.

Essential Duties:

* Establishes and maintains effective relationships with referral sources, and exemplifies excellent customer service during interactions with patients, employees, providers and others to build Specialty Pharmacy program. Implement new services and programs in response to stakeholder's needs.

* Interface and problem solve, if needed, with the referring provider and/or clinic staff; monitors satisfaction of patients, referring providers and clinical sites regarding referral process and patient care management.

* Establishes a collaborative plan annually with Operations and Development staff regarding clinical and community outreach; collaborates with other/staff team members in the implementation of outreach plan goals/objectives; implements the outreach plan to referring providers and clinics; monitors progress towards achievement of outreach plan through quarterly improvement assessments and modifying plan as appropriate. Is present at provider and community outreach events as appropriate.

* Integrate with stakeholders (patients, physicians, nurses, pharmacists, and others) to ensure that assigned Specialty Pharmacy patient receives the medications he/she needs at the appropriate time.

* Review medication regimen with pharmacists to determine which medications need to be dispensed on a timely schedule; use ability to differentiate products/services and unique value versus other options to meet needs.

* Represent on-site/off-site Specialty Pharmacy seamlessly, answer patient and provider staff questions about the service, insurance issues, etc.; assist stakeholders with data interpretation and provide recommendations for improvement of quality.

* Maintain preferred specialty drug lists in concert with inventory/shipping specialist; assume responsibility for updates internally to stakeholders.

* Monitor stakeholder concerns, as well as patient charts and profiles in an effort to meet Specialty Pharmacy therapy needs. Influence and motivate others to coordinate projects , resources and people to achieve the implementation of Specialty Pharmacy services.

Hours: 0800-1630, Monday-Friday

Required Education for Staff Job Levels

Combination of education and related experience

Required Licensure and Certifications

Current/active registration with the Nebraska Board of Pharmacy and Nebraska accepted form of pharmacy technician certification

Required Minimum Knowledge, Skills and Abilities

Relationship building, ability to work effectively with staff, payers and patients. Customer service, communication skills, computer skills and Microsoft Office suite, problem solving skills, Specialty Pharmacy therapies knowledge, knowledge of IT applications or ability to efficiently learn (e.g. EMR, dispensing, EPIC Hyperspace and/or Willow Rx, internet, Medicare remote and other on-line insurances), office management, process improvement, knowledge of government (Medicaid/Medicare) and third party payer contacts and provider agreements, ability to train others

PREFERRED Qualifications

2 year degree in healthcare or related field
