JOB SUMMARY: The APN/RNP is a licensed professional nurse that works under the supervision and direction of the Medical Director; they provide direct, comprehensive, preventative and therapeutic healthcare to patients and families including assessment, diagnosis, treatment, prescription, counseling and instruction in compliance with Nurse Practitioner Protocols and all regulations governing RNP's. The APN may provide healthcare for which the APN is educationally prepared and for which competence has been attained and maintained. Perform other duties as assigned.

ESSENTIAL FUNCTIONS:

Patient Assessment

Accurately assesses patient conditions and documents effectively to support charges and to communicate with other providers.

Provides comprehensive patient assessment, including functional, mental, social and physical conditions. Establishes accurate diagnosis.

Seeks consultation/supervision of physician as needed to confirm proper assessment.

* Patient Treatment

Independently implements and documents patient care treatments and maintains skills requisite to the position for which he/she is placed.

Utilizes multidisciplinary team services in treatment plan.

Appropriately involves patients and caregivers in treatment planning.

Provides interface between clinic and other providers to assure timely, comprehensive services for patients.

Independently develops and implements treatment plans and evaluates patient's outcome.

Complies with all statues and regulations governing Advanced Practitioner Nurses and functions within the legal boundaries of the APN category.

Responds in appropriate manner during patient emergency situations.

* Program Development

Independently identifies program weaknesses/gaps in service and develops solutions to improving patient care and customer services.

Provides professional and community education with regard to health issues.

Participates in community services events as needed.

Takes responsibility for clearly communicating patient needs, care plans and problem resolution to physicians and other health care team.

* Supervision

Possesses knowledge and skill to provide supervision to clinical staff as needed.

May develop a preceptorship for student nurses in the Senior Health Clinic.

* Program Development

Independently identifies program weaknesses/gas in service and contributes to development of program components to insure state of the art interventions and services.

Develops/maintains policy and procedure for patient care activities.

Uses process improvement skills to contribute to team development and program growth.

Actively participates in hospital and department projects and committees as appropriate.

Participates in departmental outreach and community services as needed.

Develops training modules and presentations on health issues for staff and/or community groups.

* Clinic Duties (if assigned to clinic)

Triage calls to the clinic and perform assessments for in-patients.

Return test and lab results.

Provide RN services approximately 50% of the time.

Provide clinical support for physicians in satellite clinics.

Provide care to nursing home patients approximately 50% of the time.

Share day/night call for nursing homes every other week. Take call 25/5, off at 5:00pm Friday.

Provide continuing education and skills assessment for clinical staff.

Develop provider caseload as volume permits.

Share call for holidays and vacation.

Prepare to develop a sub-specialty within the clinic that will enhance volume and comprehensive patient care.

Develop patient education programs on topics such as Nutrition.

7. JointAcademy duties (if assigned to Joint Academy)

Education: Current RNP and/or APN licensure in Arkansas. Documentation compliant with the State Nursing Board of satisfactory completion of a nurse practitioner educational program and completion of preceptorship. Eligibility for a Medicare provider number and DEA registration.

Experience: Experience in APN/RNP patient care preferred.

Physical Requirements: This job requires, at various times, the following physical activities; climbing, balancing, stopping, kneeling, crouching, reaching, standing, carrying, walking, pushing, pulling, lifting, finger dexterity, grasping, feeling, talking, and hearing, sometimes exerting in excess of 20-100 pounds of force. The worker is required to have close visual acuity to perform daily activities. May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients.

Skills: Knowledge to independently provide direct, comprehensive, preventative, and therapeutic healthcare to individuals and family members using the Nurse Practitioner Protocols/Advanced Practice Nurse. Knowledge and ability to perform technical and routine clinic procedures. Must have excellent verbal and written communication skills. Employee's work must be efficient and accurate. Must function effectively as a member of a multidisciplinary team.
