Job Summary/Essential Duties:

* Gathers relevant information systematically; considers a broad range of issues or factors; grasps complexities and perceives relationships among problems or issues; seeks input from others; uses accurate logic in analyses.

* Assists with the development, assessment, implementation, and monitoring of a comprehensive plan of care for identified populations by coordinating an interdisciplinary team approach to service and resource delivery. The assessment will include patient goals, as well as biophysical, psychosocial, environmental, and discharge planning needs. Maintains flexible work hours to accommodate patients and physicians which often includes long hours and unscheduled shifts for emergency cases and peak periods of activity.

* Serves as an advocate and liaison between patient/family and physician, hospital staff, members of the health care team, community resources, and third party payers as necessary.

* Consistently evaluates the appropriateness of levels of care, diagnostic testing and clinical procedures, quality and clinical risk issues, and provides timely, accurate and complete medical record documentation and data collection which includes delay days.

QUALIFICATIONS:

Experience:

Preferred: Three years of progressive clinical experiences as a registered nurse or licensed therapist; Two years experience with disease specific population for a specialized care manager. Strong emphasis on critical thinking, diagnostic testing and behavioral procedures.

Education/Licensure:

Required: Completion of a BSN Registered Nurse program in an accredited institution. Current RN licensure in the State of Nebraska or the State of Iowa. Nebraska and Iowa are compact states so you must be licensed in the state of residence.

OR

Master Degree in Counseling or Social Work. Must have a current license/certification in one of the following areas:

Licensed Mental Heath Practitioner (LMHP) in Nebraska

Licensed Mental Health Counselor (LMHC) in Iowa.

Licensed Independent Social Worker (LISW) in Iowa

Licensed Master Social Worker (LMSW) in Iowa

Licensed Social Worker (LSW) in Iowa

Licensed Marital and Family Therapist (LMFT) in Iowa

Certified Master Social Worker (CMSW)

Certified Social Worker (CSW)

Provisional Licensed Mental Health Practitioner (PLMHP)

Licensed Alcohol and Drug Counselor (LADC) in Nebraska

Provisional Licensed Alcohol and Drug Counselor (PLADC) in Nebraska

Advanced Certified Alcohol and Drug Counselor in Iowa (ACADC)

Certified Alcohol and Drug Counselor in Iowa (CADC)

Provisional Certified Alcohol and Drug Counselor in Iowa (PCADC)

International Alcohol and Drug Counselor -- must be approved by Iowa or Nebraska (IADC)

International Advance Alcohol and Drug Counselor must be approved by Iowa or Nebraska (IAADC)

* ***License must be held in the state you provide services.

Will obtain the following within six (6) months of employment:

* CPI Certification

* Mandatory Child Abuse Reporting (if working in Iowa)

BLS or BLS instructor certification is required upon hire. Acceptable credentialing bodies and certifications include the following: American Red Cross: CPR/AED for the Professional Rescuer, American Safety and Health Institute: CPR, PRO, must say "Professional Level-with AED", and American Heart Association: Basic Life Support for Healthcare Providers.

Knowledge/Skills/Abilities:

Job requires basic to moderate skills in reading, verbal and written communication. Strong leadership and critical reasoning skills in clinical decisionmaking. Planning and organizing, initiative and ingenuity, creativity, independent decisionmaking, problem solving, and ability to interpret data. Public relations and promotion of positive morale are important. Limited computer skills are necessary.
