About this job

Under the general direction of the Clinical Manager, the Licensed Vocational Nurse assists in the nursing process by instructing patients and care givers in self care and providing basic nursing care as needed.

Primary Job Responsibilities:

* provides direct nursing care according to the medical plan of treatment and nursing care plan

* utilizes physical and psychosocial assessment skills, performs multi-system assessments and identifies current and potential care needs

* carries out physician orders for treatment and visit frequency per plan of treatment

* organizes and completes patient treatment and tasks per established care plan

* implements and updates care plan in collaboration with the Clinical Manager and PCC to reflect progress toward goals and outcomes

* educates and trains patients and caregiver on exercises and equipment needed to aid recover

* provides education and instruction specific to patient and caregiver assessed needs, abilities and readiness

* accurately documents ongoing assessments, clinical findings, skilled care and plan for next visit according to the established care plan

* must be flexible with schedule including, but not limited to, the ability to participate in on call and/or department rotation for night and weekend coverage

Education

Graduate from an accredited vocational school of nursing or equivalent.

Current California LVN license and Blood Withdrawal certification.

Must possess current BCLS CPR certification.

Experience

Minimum 1 year of professional experience strongly preferred.

Home health or community health experience preferred.

Knowledge

Basic competencies in the fundamentals of nursing and pharmacology are required.

Skills

Demonstrated skills in effective written and verbal communication.

Ability to recognize the needs and concerns of diverse groups of people.

Ability to maintain harmonious constructive working relationships with internal and external customers. For example, patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources.

Ability to use and give instruction of adaptive equipment.

Must possess a current California Driver License and access to a reliable automobile with proof of liability insurance.

Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter Care at Home's strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor.

Must be able to deal with challenging work environment with time demands and occasional conflicting priorities.

Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.

Responsible for maintaining all required licensure and certifications.

At orientation and annually thereafter, must be able to meet Sutter Care at Home core competencies for position and area of specialty, as appropriate.
