Summary:

1. The licensed and credentialed Nurse Practitioner (NP) provides assessment, diagnosis, treatment, consultation and follow-up in collaboration with a physician as dictated by state licensure requirements. The NP provides age and specialty appropriate routine medical care and management of the patient with acute and chronic complaints, as defined by his/her scope of practice and education.

Responsibilities:

Coordinates patient care activities and the flow of patient information by effective communication with other office staff, physicians, patients, families and a variety of health care agencies and appropriate documentation to ensure continuity of patient care.

1. Demonstrates critical thinking in prioritizing patient care.

2. Provides and discusses treatment options with patients and family members

3. Maintains communication with family members

4. Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care

5. Discusses appropriate information regarding patient care with the collaborating or supervising physician

6. May dictate follow-up letters to referring healthcare providers with summaries of treatment, response, and plan according to specialty area as appropriate

7. Effectively communicates and interacts with patients and families from diverse backgrounds

8. Establishes relationships with professional and/or other health-related groups within the organization and community.

9. Shares clinical expertise with professional and supportive personnel

10. Collaborates with other health disciplines to ensure continuity and quality of care

11. Uses interpersonal skills to work productively with all levels of hospital personnel

12. Assists with orientation of new staff when appropriate.

13. Conducts appropriate and timely documentation of patients concerns, problems and/or symptoms in medical records.

14. Serves as a preceptor to NP students.

15. Requests written consultation from physicians or other healthcare professionals to ensure appropriate and quality patient care

16. Participates in staff development and in-service programs

Ensures maintenance of a therapeutic environment that promotes effective and cost efficient patient care and provides for the safety of patients, staff and visitors. Coordinates and participates as an effective member of the health care team to ensure high quality care and patient satisfaction by providing a professional and congenial work environment for patients, office staff and physicians.

Promotes/Supports Quality Improvement initiatives within the practice setting as well as within the Medical Group and the Medical Center

2. Supports the regulatory process by assisting with the inspection preparation and maintenance of standards of care

3. Maintains provision of preventive health services (e.g. screening, risk assessment, immunizations, PAP smears, mammograms, colonoscopy, etc.)

4. Applies current concepts and evidence-based findings from research and/or studies to practice

5. Initiates change in clinical practice based on current evidence-based concepts and findings

6. Works to improve patient outcomes through various initiatives, committees and work groups

7. Enhances the ability of others to provide health care by acting as a role model

Provides health care and demonstrates the knowledge and skills necessary for providing care as an Advanced Practice Nurse to patients in a primary/specialty care setting.

1. Demonstrates ability to function both independently and in collaboration with or under the supervision of a physician or group of physicians according to specialty area

2. Obtains complete histories and performs complete physical exams on new and returning patients and identifies deviations from normal in the physical assessment

3. Evaluates and treats patients with acute and chronic conditions related to the specific specialty area

4. Formulates a diagnosis and treatment plan based on the assessment

5. Performs, requests, and/or analyzes diagnostic studies as indicated upon evaluation of the patient and assures follow-up of test results if necessary

6. Collaborates with physicians as appropriate.

7. Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the patient

8. Completes written or verbal documentation of patient history, admissions, care plans, progress, discharge notes, etc. in the patient's medical record

9. Documents and reports care in accurate, timely manner using appropriate forms and records

10. Demonstrates ability to assess and anticipate patient needs.

11. Handles phone/electronic messages and pages in a timely and appropriate manner.

12. Accurately records telephone conversations in the medical records.

13. Maintains certification by attending the appropriate seminars and conferences.

14. Continues to keep informed of current standards of practice for nurse practitioners.

15. Possesses knowledge and skills necessary to obtain adequate lab specimens for testing.

16. Performs in-office patient lab testing and quality control in compliance with medical provider's orders and office lab procedures/policies.

Provides patient education through application of teaching-learning principles appropriate to the age and needs of the patient population served by the office. To help patients prevent and/or effectively cope with medical problems and to promote a healthy lifestyle.

1. Demonstrates the knowledge and skills of age appropriate care.

2. Assesses information and data indicative of the patient's learning capabilities.

3. Interprets information relative to the age-specific needs of the patients.

4. Promotes effective communication by providing effective learning techniques to patient and family members.

Other information:

Education:

1. Master's Degree in the field(s) of Nursing Required

Credentials:

1. CPR

2. Registered Nurse in the state of IL

3. Certified as APN. Certified by ANCC or AANP or other appropriate Certifying body. Must be credentialed by the MMG Credentialing Committee.

4. Licensed in Illinois as Nurse Practitioner.

5. Other: Must have physician/nurse practitioner collaborative agreement according to the IL Nursing Practice Act, Nurse Practitioner. Must have DEA number and controlled substance license number, as per the prescriptive authority delegated by your collaborative physician.

Experience:

1. 1-3 years of experience in/with Nurse Practitioner preferred.
