Position Qualifications:

* We require a nursing degree from an accredited school.

* You'll also need a current RN license in our state.

* Basic life support training should be completed in 90 days with advanced training required at certain intervals.

Responsibilities:

Critical Care Registered nurses are accountable for the provision of nursing care assigned patients in all phases of care in ICU. They are responsible for the prescribing of nursing interventions, the coordination of patient activities, and delegation of those aspects of care, which may be provided by other members of interdisciplinary team. Basic to the accomplishment of this goal is individual professional accountability through adherence to standards of nursing care of critically ill, and through a commitment to act in accordance with ethical principles. The nurse will perform all work in accord to the mission, vision and values of Mercy Memorial Health Center.

Assesses the condition of patient/family/significant other in a timely, consistent and systematic manner. Documents objective and subjective data according to policy.

1. Initiates a nursing assessment, performs an orientation and obtains admitting orders, within one hour of admission.

.2. Completes assessment for each patient according to policy and more frequently as indicated by patient's clinical condition.

3. Documents patients heart rhythm on the medical record and notifies patient's primary physician or cardiologist when changes occur.

4. Utilizes Critical Care nurse tech and other appropriate members of the health care team in data gathering function of the nursing assessment.

5. Includes information obtained from the patient/family/significant other in the assessment. Documents clearly and concisely initial and ongoing assessment data about the patient/family/significant other in the permanent record.

6. Communicates subjective and objective data about the patient/family/significant other with appropriate members of the multidisciplinary team.

7. Assesses and appropriately documents the education needs and learning abilities of patient/family/significant other during the admission and subsequent assessments in the computerized care plan.

Develops an outcome oriented plan of care that reflects the patient's actual or potential health problems.

1. Formulates care plan within 8 hours of admission and revised it every 24 hours as needed.

2. Formulates a plan of care including identified problems, and/or nursing diagnosis, nursing interventions, and goals/outcomes mutually set with patient/family/significant other.

3. Formulates a plan of care based on patient's current problems, congruent with the medical plan of care, and incorporates preventive, therapeutic, rehabilitative, palliative, and comforting nursing interventions.

4. Prioritizes care delivery according to the patient's needs and unit activities.

5. Collaborates with the multidisciplinary team when appropriate to revise the plan of care, including discharge planning.

6. Demonstrates the ability to obtain and interpret information in terms understandable to the adult and geriatric patient.

7. Plans care based on knowledge of changes associated with aging in the adolescent, adult and geriatric patient population.

Implements the plan of care to achieve the identified outcomes for the patient according to policy and procedure.

1. Provides professional nursing care to an assigned group of patients according to the plan of care.

2. Assists with and/or performs procedures including but not limited to the professional Critical Care nurse skills checklist.

3. Effectively communicates, delegates and prescribes patient care tasks for Critical Care MPE in accordance to their job description. Carefully monitors their performance and patient outcomes. Provides feedback to the charge nurse.

4. Will meet the patient's biopsychosocial and educational needs including the safe use of equipment used in treatment or health maintenance.

5. Demonstrates the ability to provide care and information based upon physical, psycho/social, cultural, safety and other age-related factors for the adult/geriatric patient.

Evaluates effectiveness of services provided.

1. Evaluates patient/family/significant others responses to care; determines progress towards achievement out outcomes at least every 24 hours.

2. Evaluates patient/family/significant other's understanding of information provided or skills taught.

3. Evaluates the effectiveness of discharge plans for meeting the needs for continuing care.

4. Participates in the evaluation of the efficacy of unit and organizational services in meeting the needs of the consumer and/or the community.

Demonstrates effective and accurate verbal, written and electronic communication.

1. Creates a positive climate for communications.

2. Maintains the confidentiality of patient, staff and hospital information from written, verbal electronic sources.

3. Communication is timely, accurate, concise and complete.

4. Utilizes constructive problem solving techniques to promote change and deal with issues. Follows established channels for addressing issues, concerns and procedures.

5. Documentation of the Nursing Process is completed according to organizational policies and procedures.

6. Maintains relevant knowledge of hospital forms, completes in a timely, accurate and complete manner (i.e. Quality Improvement reports, employee/patient incident reports).

7. Demonstrates skilled use of the Nursing, OE, PCI and QA modules as indicated below.

Participates in unit and organizational activities.

1. Participates in 75% of staff meetings.

2. Participates in Process Improvement activities such as data collection, in-services, meetings, writing plans, etc.

3. Participates in a hospital committee, special project or service (i.e. preceptor to student or new employee, care management) as requested.

Comply with organizational policies and procedures.

1. Maintains current certification in BLS.

2. Successfully completes ACLS within first year of employment in critical care, is maintained throughout employment.

3. Attends all mandatory in-services.

4. Complies with all organizational policies, procedures and protocols no documented violations.

5. Adheres to OSHA standards regarding appropriate safety regulations and procedures, including fit testing for appropriate filtration mask yearly and/r needed

6. Completes Critical Care Annual Competencies.

7. Completes a minimum of five (5) shifts of orientation within the first four weeks of employment and completes the Critical Care Checklist.

8. Accurately completes clocking transactions.

9. Maintains hospital property and security systems to ensure a safe environment.

0. Maintains an environment of confidentiality for all patients and hospital staff.

11. Completes equipment checks as assigned.

Demonstrates professional conduct and development.

1. Uses current knowledge achieved through research or educational activities to enhance or improve clinical practice or patient care.

2. Participates in unit development and decision making through such activities as attending staff meetings, serving on a committee or task force and/or special project work.

3. The Critical Care Nurse will be responsible for individual professional growth and development, demonstrating that growth by: 1) ability to care for more complex patients and daily assignments; 2) ability to independently research complex patient care issues and consistently act as a resource person for colleagues; and 3) effectively sever as role models and/or preceptors.

4. Establishes goals as role models, and/or preceptor's appraisals, provides evidence of goal attainment from previous evaluation period.

Unit Specific Responsibilities.

1. The nurse whose primary cost center is ICU shall: 1) demonstrate competence in the skill of pulling venous and arterial sheaths: 2) demonstrate competence in the skill of performing ACT's; and 3) demonstrate competence in the skill of caring for patients requiring a pacemaker. 4) demonstrate competence in the concepts of invasive and non-invasive ventilators: 5) demonstrate competence in the skill of ABG interpretation.

2. All ICU nurses shall have the opportunity to complete an intra-aortic balloon pump and CVVHD class and care for patients requiring that technology.

3. All special skills should be accomplished within the first year of employment in the ICU and will be tested on an on-going basis.

4. Completes continuing education specific to Critical Care Nursing as warranted.
