Come join a team that impacts families by offering a service rich environment of advocacy and love.  Our prevention program serves families at risk of out of home placement.  Our goal is to preserve the family unit by providing supportive casework counseling in the home.  We provide case management services; conduct clinical assessments; identify service needs; develop treatment service plans with the family. We provide referrals to community services; communicate with collaterals to monitor and track progress.  We empower parents with tools and resources to ensure their children's safety, well- being, and permanency. We provide advocacy to ensure quality services; and document our efforts in a web-based case management program.  Agency training and supervision provided to case managers for ongoing professional development and support.

 

 

Required Knowledge, Abilities and Skills

 

  • Minimum of a Bachelor's degree in Social Work, Human Services or related field
  • Minimum 3 years of experience working with at risk children and families; providing case management services in the home.
  • Ability to develop treatment service plans; summarize all client contact in progress notes.
  • Ability to manage office work time to complete assigned tasks and meet deadlines.
  • Ability to advocate on behalf of the child in the school while increasing students' and parents participation in school activities; improve school attendance.
  • Experience with developing relationships with schools, medical providers, mental health programs, substance abuse programs, domestic violence programs, family shelters, community partners, and Department of Social Service agency is a must.
  • Ability to form engaging and therapeutic relationships with children and families; clinical interviewing skills a must.
  • A clean, valid driver's license is required. Local travel throughout Dutchess County is required weekly.
  • Ability to provide on-call coverage; flexible schedule with evenings as needed to service working families
  • MS office Skills required
