Position Summary

Responsible for marketing services and securing referrals from facility based discharge planners, payers and physicians. Optimally maintains the quality image of PSA Nursing within the community.Work with our referral sources in assisting discharge planners, clients and their families with the transition from facility to home based care. The Clinical Nurse Liaison uses their clinical knowledge and knowledge of state homecare regulations to work with the regional PSA Healthcare locations to insure a positive outcome for the clients, families, and case managers. The Clinical Nurse Liaison will also work to foster strong and positive relationships with existing and new referral sources. Liaison nurse professionals must have excellent communication skills, as they will need to form strong relationship with key hospital and medical personnel.

Job Responsibilities

* Market services to current and new referral sources and establish relationships within key facilities.

* Assume responsibility in coordinating care to newly referred clients, establishing a relationship with families and clients

* Developing sound working relationships with hospital staff in order to facilitate the homecare referral process.

* Educating hospital staff regarding the benefits of home healthcare services and appropriate patients for home healthcare

* Keeping Location Directors informed of changes in healthcare community and making recommendations on ways to meet company goals while maintaining compliance with state and federal guidelines.

* Establishing linkage with the community to maintain awareness of community needs.

* Presenting in-services to hospital staff, physician's office, and other healthcare facility staff, as requested.

* Demonstrating professional behavior and serving as a role model to team members at all times

* Monitoring and gaining awareness of competition activity in market

* Attending industry and professional meetings as appropriate

* Provide direct client care as needed

* Evaluate the quality and effectiveness of nurse practice and nursing services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and patient outcomes

* Provide nursing updates and assist in obtaining authorization for care

* Identify professional practice standards within the organization and identify areas of strengths as well as areas for professional practice development

* Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice

* Maintain a professional demeanor consistent with registered nurse standards of practice

* Provide best practice in delivery of nursing care to the appropriate population and adhere to the standards of professional nursing practice

* Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to socio-cultural diversity

* Perform other duties as assigned by supervisor

Minimum Qualifications

* Proof of eligibility to work in the United States

* RN licensure in designated states as appropriate

* Knowledge and understanding of compliance with adherence to regulations

* Diploma, Associate, or Bachelor degree in nursing from state accredited RN program

* Strong commitment to clinical excellence

* Ability to problem solve

* Ability to resolve conflicts

* Ability to assess clients and provide direct client care as needed

* Good organizational and communication skills

* Ability to prioritize and multi-task

* Possess critical thinking skills
