Shift: DAYS

Position Summary

We are seeking an individual who under pharmacist supervision and according to established procedures will assist in the admission medication reconciliation process. They will conduct initial patient, family, or primary caregiver interview in regards to medication use. The outcome of this interview is formulation of a complete and accurate medication list that is ready and available for physician review in a timely manner. Adherence to all privacy policies and procedures such as HIPPA regulations is essential.

Education Requirements

High school diploma or equivalent.

Work Experience Requirements

1 year hospital or retail pharmacy experience as applicable (completion of an Accredited Technician training course may substitute for experience).

Working knowledge of computer systems.

Licenses/Certifications

Pharmacy Certification.
