MA Care Coordinator-Utilization Management-Redwood Shores - Full-time / Part-time

Under minimal supervision, the care coordinator is responsible for the intake coordination of Accountable Care Organization (ACO) members, working closely with the Managed Care team and other stakeholders including, but not limited to physicians and other applicable staff. The care coordinator receives referrals from health plans and prescreens new referrals and intakes the information, organizing, inputting, and reporting on the patient registry daily or as needed.

REQUIRED MINIMUM QUALIFICATIONS:

Certified Medical Assistant

High school graduate or completed GED

At least one year post high school education (junior college or university)

Three years related work in the healthcare field

PREFERRED:

Successful completion of 4-year college degree in Health Administration, Business Administration, Life Sciences or a related field

5 years related work in the healthcare field

3 years of clinical experience in healthcare setting

1 years related work in a Managed Care setting

Working knowledge of EPIC
