HOME HEALTH AIDE
- Provide personal care and light housekeeping services to patients in their home.
- Observes, reports and documents services regarding the patient.
- Provides ADLs, feeding, assisting with exercises, ambulation, transferring and activities.
- Accompanying patient on medical appointments, shopping and running errands.
- Work in cooperation with family, the nurse and case manager to support the patients in meeting their needs.
EDUCATION / EXPERIENCE / CERTIFICATION
- High school diploma or equivalent
- CPR & First Aid certification
- Current physical and PPD
- One (1) year of direct patient care experience preferred
- Valid NJ driver's license
- must have use of automobile in good working condition
- Satisfactory criminal background check
