SUMMARY:

Clinically supervise up to 5 MSW-level Psychiatric Social Workers. Provide Mental Health Crisis Intervention, Social Casework, and therapeutic services to a caseload of an older person with psychiatric disturbances, dysfunction, and/or impairment. Provide above services in English or Spanish as necessary. 


DUTIES AND RESPONSIBILITIES: 

* Clinically supervise up to 5 MSW and BA-level social work staff, and other community aides, Title V workers, homemakers, home heath aides interns and volunteers, etc.
* Provide field instruction to undergraduate or graduate students in social work or other disciplines as assigned.
* Participate with medical, psychiatric, nursing and social services staff in the process of interdisciplinary team assessment of the mental heath, social and emotional needs of older adult client(s) and their families.
* Administer Mental Status Examinations to older clients so as to screen for possible disorders and impairments requiring mental heath services. 
* Participate in interdisciplinary team conferences, reviewing treatment and case plans, case monitoring and discharge planning, pursuant to sound professional practice to federal, state, and city guidelines.
* Responsible for the provision of the full range of psychiatric social services to a specialized borough-wide caseload of psychiatrically impaired older adult clients(s) and their families, including:

  • In the capacity of Primary Therapist, to develop, implement, monitor and document treatment plans, coordinating the activities of JASA staff and related personnel and involved agencies in the management of that plan
  • To provide individual and family psychotherapy in the resolution of problems of the client(s) and family.
  • To provide appropriate information to clients and collaterals regarding the normal physical, social and psychological development of individuals, challenges to functioning presented by conditions of impairment, disease, social stress and dysfunction and gero-psychiatric disorders; and way socoping and preserving individual functioning and autonomy.
  • To provide specific information about health, welfare, education and recreation services available to serve the older client, with particular reference to resources for gero-psychiatric clients.
  • To arrange for direct provision of special services such as homemaker, home health aide, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, vocational placement, medical and psychiatric examination and therapy housing, etc.
  • To evaluate clients' capacity to manage their own affairs and protect themselves from financial exploitation. To assist in securing needed and legitimate financial management or guardianship arrangements. Disbursement of monies as issued through JASA relief funds or client financial management services, to secure needed goods and services for older clients.
  • As appropriate, make referrals to and maintain cooperative relationships with public and voluntary agencies, program, services and facilities.

* Under psychiatric supervision, conducts group psychotherapy services for clients and families. 
* Participate and provide leadership in staff committees, program planning and conferences and meetings.
* Represents the agency at community conferences and meetings.
* Accurately completes required case records, reports and statistics.
* Performs other duties as assigned.

QUALIFICATIONS:
* Graduate of an accredited School of Social Work with MSW degree and clinical concentration.
* At least three years of Social Work experience in geriatric and/or psychiatric setting
* New York State Licensed Clinical Social Worker (LCSW), active or pending.
* Possess an NPI number.
* Possess a Medicare Provider Number.
* Ability to read, write, speak and understand English and Spanish.
* Good writing/record keeping skills.

* Must be computer literate, including the ability to learn and use program-specific software, and internet savvy.
* Ability to travel to clients' homes using mass transit and to negotiate any steps and barriers in clients' residences.
* Knowledge of Yiddish/Russian/Polish may be helpful.
* Understanding and appreciation of the roles of race, relating, ethnicity, sexual preference and individuals values as they relate to serving clients and families.
