Clinical Pharmacist

Job Responsibilities

As a Clinical Hospital Pharmacist, you will review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Y ou will maintain liaison relationships with medical and nursing staff to improve drug usage and therapeutic outcome.

Additional responsibilities of the Clinical Hospital Pharmacist include:
  • Responsible for setting criteria for medication analysis, coordinate reports to clients, meet regulatory reporting requirements for drug utilization review and fraud and abuse reporting.
  • Develop clinical criteria for review of non-formulary or non-preferred drug lists medications and communicate recommendations to clients
  • Oversee the drug utilization review and medication therapy management programs at the assigned plan to meet
  • Develop and design clinical edits to adjudicate claims and enhance drug utilization
  • Provide clinical guidance and support to the assigned pharmacy technicians
  • Develop and review guideline criteria for prior authorization and non-formulary drugs in support of Pharmacy and Therapeutics Committee function and pharmacy benefit design.
  • Ensure and evaluate quality controls and implement opportunities for performance improvement in pharmacy practices
  • Research drug information to maintain drug knowledge and disease state understanding for application to prior authorization reviews
  • Providing specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses
  • Coordinating the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions
  • Ensuring compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records
  • Mentoring pharmacy interns in training including but not limited to Pharmacy Practice Residents and pharmacy students; activities should include both practical and didactic experiences
  • Conducting and evaluating medication histories, assessing compliance and suggesting modifications so as to achieve desired outcomes
  • Instructing patients in the proper use of prescribed drugs; participating in patient care rounds with multidisciplinary teams to evaluate patient progress
  • Individualizing medication regimens using sound principles, accounting for pharmacodynamic/pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility
  • Conducting and participating in research including, but not limited to, performing as the primary investigator or co-investigator for research programs which will impact on the delivery of quality care or examine the pharmaco-economic impact of providing care to the patients

Specific Inpatient Pharmacist requirements:
  • Area: Ability to work in the Modesto, CA area
  • Licensure requirements: Must have valid, open and active California Pharmacist license
  • Education : B.S. or Pharm D
  • Years of experience: 2-3+ year(s) of recent pharmacy experience within a hospital setting
  • Schedule: Full time or part time- Shift will vary
  • Length of Assignment: Temp to hire OR Per Diem OR Direct Hire- Depending on candidate and their availability
  • Pay: Depending on experience ( DOE)
  • Background/drug requirements: Ability to successfully complete background check, client health screening criteria and drug screen
