Beacon Hospice is currently seeking a Full Time/PRN Hospice Nurse. As a member of the multidisciplinary team, the primary nurse is responsible for coordinating all aspects of the client's nursing care. Facilitates patient care activities by providing advice, support, and assistance to the IDT members as needed. Responsible for ongoing patient/family evaluation and assessments. Develops and maintains a written care plan for each patient and updates as needed and with re-certifications. Maintains complete, accurate, and up to date medical records. Documents all contacts with patient and family unit and has documentation in office per policy. Collaborates with the IDT members to ensure the best care for the patient/family unit according to the Beacon Hospice philosophy of care. Required experience:
- Previous home health, hospice, or comparable setting experience.: 1 year
Required license or certification:
- Current SC License as Registered Nurse in good standing
