Location: New Iberia, LA

Responsibilities:

  • Conduct comprehensive patient assessments and develop customized care plans.
  • Deliver skilled nursing interventions, including wound care, medication management, and patient education.
  • Collaborate closely with patients, families, and caregivers to ensure holistic care delivery.
  • Communicate effectively with the interdisciplinary team to coordinate care and address patient needs.
  • Maintain accurate and timely documentation of patient encounters.

Requirements:

  • Active RN license in Louisiana.
  • Minimum 6 months of experience in home health nursing.
  • Familiarity with HomeCare/HomeBase EMR system preferred.
  • Current BLS certification.
  • Availability for Monday to Friday day shifts.
  • Ability to travel between patient homes and meet productivity expectations.
