BEHAVIORAL HEALTH SOCIAL WORKER JOB SUMMARY:

  • MAT (Medication-Assisted Treatment) Program Specialist will provide a service that combines medication with alcohol or drug addiction treatment to prevent relapse or treat substance use disorders.
  • Assessing patients: Perform physical exams, taking vital signs, and evaluating health history
  • Providing care: Deliver MAT as appropriate, and provide ongoing care for patients' medical, physical, and psychosocial status
  • Documentation: Provide accurate and timely documentation of observations, interventions, plan of care, referrals, and follow-up
  • Referrals: Make appropriate referrals to other health professionals and community agencies
  • Coordination: Coordinate care for individuals enrolled in the MAT program
  • Support: Support individuals served and their family members through transitions and transfers between receiving facilities, housing providers, residential treatment facilities, and others

BEHAVIORAL HEALTH SOCIAL WORKER HIGHLIGHTS:

  • Shift Hours: 8am-4pm, Mon-Fri
  • Location: Onsite (no work from home options) in Karthaus PA 16845 
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
  • 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
  • The compensation for this role is $25 per hour.

BEHAVIORAL HEALTH SOCIAL WORKER REQUIREMENTS:

  • Bachelor Degree in Social Work or equivalent education/experience required
  • Knowledge and experience in counseling/therapy for drug & alcohol abuse, discharge planning and community resources.
  • Basic Computer skills
  • Ability to effectively communicate with patients, families, and hospital personnel.
