Home Health Clinical Director, RN $15,000 Bonus - Full-time / Part-time

What you’ll do in this role:

• Assists VP of Operations in planning development and administration of the Agency as set forth in the policies and procedures.
• Serves as a clinical liaison between interdepartmental teams, referral sources and patients.
• Provides leadership in promoting and maintaining standards for giving food quality of care by all members of the patients care team.
• Establishes service committees to facilitate and attain the objectives of the service program Provide supervision, direction and performance management to Assistant Clinical Directors.
• Evaluate and address customer concerns, including documentation and resolution of same.
• Communicate with marketing personnel regarding customer issues that arise and may
  affect referral source relationships.
• Audits clinical records for completeness on an ongoing basis and works with professional clinicians to ensure compliance and comprehensive documentation.
• Works with professional clinicians in developing the plan for patient frequency of visits that are clinically appropriate for the patient.
• Assists agency personal in care coordination of patient/client services. Serves as a liaison between the organization, referral sources, patient/clients and agency personnel.

We are looking for compassionate Clinical Director with:

• Bachelor’s Degree in Nursing or Therapy and licensed in the state of Illinois.
• Supervisory experience, preferably in home care.
• Minimum of five (5) years of experience working as a Registered Nurse or Therapist in a home care setting.
•  Minimum of seven (7) years of management experience.
