Position Summary
The Case Manager will intake, coordinate, and oversee individualized care plans for pediatric patients, collaborating with healthcare professionals to ensure comprehensive treatment and support for optimal well-being.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
Providing Case Management services to clients including referral to outside providers as
needed.
Preparing and maintaining records and case files, including documentation, such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.
Monitoring treatment progress and adjust care plans as necessary.
Providing education and support to families regarding medical conditions and therapeutic interventions.
Collaborating with insurance providers to ensure coverage for necessary services.
Ensuring compliance with regulatory and legal requirements in patient care.
Collaborating with healthcare professionals to develop comprehensive treatment strategies.
Conferring with clients to discuss their options and goals so that programs and plans for accessing needed services can be developed.
Monitoring and record clients' progress to ensure that goals and objectives are met.
Performing other duties as requested.

Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in nursing, social work, or related field.
- Three to five years of case management experience preferred.
- Familiarity with healthcare regulations and compliance.
- Proficiency in medical documentation and record-keeping.
- Knowledge of child development and pediatric medical conditions.
- BLS Certification (Required)
Weekly schedule:
- Monday to Friday 8am-5pm
