Job Description

SIGN-ON BONUS!

We are seeking a dedicated Physical Therapist to join our Homecare team with immediate FT/per diem positions available. Multiple locations within Cattaraugus county. We offer flexible schedule and competitive pay with a well-established company.

Responsibilities:
- Conduct PT assessments to develop individualized treatment plans in a home-based setting
- Implement PT interventions to aid in recovery
- Develop home exercise programs and assist clients in transitioning to self-care
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Client scheduling, documentation of progress notes and treatment outcomes in electronic medical record

Requirements:
- Degree in Physical Therapy from accredited institution
- Valid NY state licensure as a Physical Therapist
