Job description

• Position responsible for both indirect and direct patient care using professional nursing skills and knowledge.
• Analyzes health status of patients in determining nursing care needs, identifying goals and patient nursing care plans.
• Responsible for ensuring quality care services are delivered to the patients.
• RN has oversight of her caseload and all RN, LPNS that work with the patients on her caseload. This is placed into territories.
• Position requires second-tier problem solving and compliant resolution while maintaining high standards of ethics and integrity, including good communication, confidentiality, compliance with laws and company policies, and professional conduct.
• RN Case Manager reports to the IDD, Home Health and Nursing Executive Supervisor

Primary Responsibilities:

• Evaluates the effectiveness of the quality of nursing care provided.

• Participate in the development of an appropriate individualized care plan based on age, cultural and spiritual values.

• Communicates regularly with management, providing status and activity reports as agreed.
• Completes all required reports and documentation.
• Performs other job-related duties as may be assigned.

Experience:

• Nursing: 3- year (Required)
• Case management: 1 year (Preferred)
• License/Certification:
• RN License (Required)

Willingness to travel: - 25% (Preferred)
