JOB SUMMARY

Under the direct supervision of the Operations Director, the PACE Home Care Coordinator is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment (DME) and community-based care.

REQUIREMENTS

1. Bachelor of Science in Nursing.
2. Current RN license in the state in which the PACE center is located.
3. At least 1 year RN with home or community health provider experience.
4. Supervisory experience desired.
5. 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
6. Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
7. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
8. Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
