Requirements:

  • Valid and active license issued by the Physical Theraphy Baord of California.
  • At least 12 consecutive months of performing similar services within the last 3 years.
  • CPR and BLS certification from American Heart Association (AHA) or Red Cross (with AED).
  • Must be proficient in the English Language (Speaking/Writing).

Responsibilities:

Under Institution CEO/CME's guidance, the Physical Therapist duties/responsiblities shall include, but not limited to:

  • Providing care per licensure scope and Institution/Facility policies, in line with evidence-based Physical Therapy practices.
  • Offering Physical Therapy and evaluations as needed, based on scheduled appointments by the Institution/Facility CEO/CME or designee.
  • Delivering Physical Therapy to patients/youth via physician referral, orders, or direct access for documented diagnoses.
  • Conducting initial assessments, forming treatment plans, and communicating effectively with patients/youth and medical staff.
  • Completing reports, assessments, and progress notes using CDCR-approved forms in patients' records.
  • Being reachable for phone consultation during Institution/Facility duty hours.
  • Maintaining rigorous daily charting; overdue charts beyond ten (10) days might end the Vendor Worker's engagement.
