Position Overview:

As a Service Navigator, you will be a vital part of the Aging Answers team. Your role involves providing personalized assistance to individuals and groups, primarily through telephone interactions. You will guide clients and caregivers in accessing essential services related to health insurance, Medicare, Medicaid, Long-Term Care, and other community resources. The goal is to alleviate pressure on existing staff, enhance workflow efficiency, and prevent costly interventions like institutional placements.

Responsibilities:

  • Complex Navigation Support: Assist clients in navigating complex systems related to health and social services. This includes understanding and explaining eligibility criteria, application processes, and available options.
  • Application Assistance: Help clients complete necessary forms and applications for various programs.
  • Case Management: Provide case management services, ensuring continuity of care and timely follow-up.
  • Options Counseling: Offer informed guidance on available services, empowering clients to make well-informed decisions.
  • Person-Centered Counseling: Utilize person-centered approaches to understand clients’ unique needs and preferences.
  • Short-Term Support: Address urgent needs promptly, ensuring timely interventions.

Qualifications:

  • Education: A master’s degree in social work or a related field in Human Services is strongly recommended. However, candidates with equivalent education and experience will also be considered.
  • Experience: A minimum of three years of relevant experience is preferred.
  • Certifications:
    • AIRS Certification: The Service Navigator should either hold AIRS certification or be eligible to sit for the certification.
    • SHIP/CHOICES Certified: Familiarity with SHIP (State Health Insurance Assistance Program) and CHOICES (Connecticut’s program for Health insurance assistance, Outreach, Information, Counseling, and Eligibility Screening) is advantageous.
    • Case Management Experience: Prior experience in case management is essential.
    • Resource Training: Commitment to participate in two-hour resource training sessions monthly.
