What You Should Know About the RN/LPN Admission Care Liaison Role:
  • The Admission Care Liaison is a DRIVING position supporting Wilmington, Fayette County areas
  • Schedule hours are 10am-8:30pm, with no weekends and 1-2 holidays per year
  • We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The RN/LPN Admission Care Liaison Essential Duties Are:

Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN/LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN/LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.

RN/LPN Admissions Care Liaison Qualifications:

RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred.

LPN's - Registered LPN with the State of Ohio without any board actions. One-year nursing experience preferred.

  • CPR Certified
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
