Physician - Per Diem SNF Coverage

Position Details:

Hartford HealthCare is seeking a per diem Internist, Family Medicine or Geriatrician on a per diem basis to provide Skilled Nursing Facility (SNF) rounding and coverage.   

• Role: Geriatrician
• Location(s):  Northwest, CT (Litchfield County)
• Setting: 100% Skilled Nursing Facility (SNF) – patient admitted and rounding
• Schedule:  Per Diem, as needed (seeking 4-5 hours/day, 3-4 days a week, flexible)
• Call:  No call

• Supported by team of APP’s that are in the facilities full time within a team of 18 Geriatric providers
• Strong support staff, nursing and operational team plus strong peer support to assist with any challenging situations that may arise
