BASIC FUNCTION: Responsible for implementation of the goals and objectives of the unit, nursing service and facility through coordinating resident care management and staff direction and by providing direct nursing care to residents. RESPONSIBILITIES/TASKS: NOTE: Some descriptions may have more specific duties and/or goals and objectives attached to this form. Such attachments normally reflect unique aspects of specific locations, shifts, departments, etc.

*1. Ensure that the individual health care plan is followed; administer or supervise treatments prescribed by physicians.

*2. Maintain compliance with all Buckner policies, procedures and requirements. Maintain compliance with all federal and state laws and all agency regulatory guidelines and requirements.

*3. Make meaningful rounds of all residents, utilizing health care plans; make rounds with physicians and accurately and timely record visits on charts.

*4. Administer intramuscular and subcutaneous injections; perform in-house testing procedures; take and record vital signs.

*5. Assist in maintaining current health care plans according to a schedule rotation and documenting acute episodes or significant changes in resident status.

*6. Supervise aides in the performance of their duties by monitoring their work; assign aides to residents, specifying break times and special duties to be completed.

*7. Meet with director as needed and participate in staff meetings to keep staff performing at optimum level.

*8. Personally receive or place all calls to physician and follows through with physician's orders timely and accurately.

*9. Supervise (or personally handle) the preparation and administration of prescribed medications accurately.

*10. Order all new medications and refills; give initial dose of all new medications and chart it timely and accurately.

*11. Order and maintain nursing supplies; monitor condition of medication room, cart and refrigerator.

*12. Direct charting on his/her shift and makes detailed evaluation of all resident charting in accordance with facility policy, regulatory guidelines and resident status.

*13. Recognize significant changes in the condition of the resident and takes necessary action as directed by Director of Nursing.

*14. Professionally communicate with resident families and respond to questions or concerns.

*15. Supervise serving of prescribed diets and fluid intake; report persistent unresolved problems to the physician and/or the Director of Nursing or appropriate discipline.

*16. Supervise the preparation of, or prepare, incident reports.

*17. Accurately and timely complete focused assessments and appropriate discharge documentation.

*18. Detect and correct situations that have a probability of causing accidents or injuries to residents or others immediately.

*19. Intervene in resident disputes or disruptive behavior, if necessary, to maintain proper facility harmony and resident safety.

*20. Accurately monitor nursing equipment, repairs, and/or replacement.

*21. Adhere to the Licensed Vocational Nurse practice standards as established by the Texas Board of Nurse Examiners.

*22. Attend and participate in staff meetings and in-service training, as required.

*23. Provide additional leadership for ancillary staff.

*24. Work schedules, assignments and units as requested by the facility to meet business needs.

*25. Accurately and timely complete documentation.

*26. Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.

*27. Perform general office tasks as required.

28. Perform special assignments, projects, and other duties as required. * Task that is considered essential functions of the job. Numbers 1-12, 13, and 21 are primary functions of the job.
