Details:

• Location: Pollocksville, NC
• Type: Full-time, Contract
• Duration: 13 weeks with possibility of extension
• Schedule: Monday to Friday, 8:00 am to 5:00 pm, with occasional weekends and on-call duty (typically one weekend per month)
• Hours: 40 hours per week
• Compensation: Weekly pay, negotiable based on territory budget and candidate experience
• Benefits: Mileage reimbursement, medical, dental, vision, 401k after 90 days

Job Duties:

• Conduct comprehensive assessments of patients' health status, focusing on physical, emotional, and environmental factors, to develop personalized care plans.
• Deliver skilled nursing care including wound care, medication management, and patient education, tailored to meet the unique needs of each patient.
• Collaborate effectively with interdisciplinary team members, including physicians, therapists, and social workers, to ensure seamless coordination of patient care.
• Utilize OASIS and Homecare Homebase software proficiently for accurate documentation of patient assessments, interventions, and outcomes.
• Monitor patient progress closely, evaluate the effectiveness of care plans, and make necessary adjustments to optimize patient outcomes and satisfaction.

Requirements:

• Active Registered Nurselicense in the state of North Carolina.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of home health experience.
• Proficiency in OASIS and Homecare Homebase.
• Reliable transportation.
