In 1964, we were established by a dedicated group of concerned citizens and government officials of Houston, Wabasha, and Winona counties. In 1965, we opened our first clinic in Winona.  Today we provide services in Winona, Houston, Wabasha, Goodhue and Fillmore counties.

TITLE:   Program Lead (Practitioner Staff) 

 

PROGRAM:   Children’s Community Based Services 

 

JOB SUMMARY:   This position entails time spent providing work direction, technical guidance and ensuring quality control.  This individual will also perform direct service to consumers in other CCBS (Children’s Community Based Service) programs for the remainder of the time. 

 

JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:    

  • Assume responsibility for direct service work in the CCBS, Children’s Mental Health Case Management Program and Skills programming, including maintaining assigned caseload of TCM Clients and/or Skills Clients.  
  • Assists the Program Director and Program Manager in the formulation, evaluation and installation of policies, procedures of Case Management and Skills Programming. 
  • Evaluate staffing and program needs. 
  • Provides non-clinical consultation to staff, other departments in the agency, and to community resources. 
  • Attend and participate in scheduled community events, staff meetings, and staff committees as assigned; as well as workshops, conferences and in-service presentations as directed by the Program Director 
  • Assume responsibility for meeting service level standards established by the Program Director. 
  • Assume responsibility for maintaining case files, which include obtaining and updating necessary releases of information, Functional Assessments and Integrated Treatment Plans. 
  • Participate in Clinical Supervision and necessary MDT Meetings. 
  • Provide assistance to clients when they are in a crisis situation. 
  • Complete all documentation as required. 
  • Take lead role in job orientation and training of new staff, as needed. 
  • Attend conferences, in-services, and training necessary to meet licensure and program requirements. 

 

 

PHYSICAL REQUIREMENTS FOR POSITION:  Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. This is primarily a sedentary position (standing and/or sitting). Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting of office materials up to 30 lbs.  

 

NON-ESSENTIAL FUNCTIONS:   Perform other duties as assigned by the Program Director and/or Program Manager. 

 

EQUIPMENT USED:   Computer, telephone, office equipment, vehicle. 

 

JOB QUALIFICATIONS AND REQUIREMENTS: 

  • Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes. 
  • Ability to work with children with a Severe Emotional Disturbance. 
  • Ability to problem solve and critically think to make appropriate decisions efficiently. 
  • Possess a high level of human relation skills. 
  • Effective organization and time management capabilities 
  • Enhanced written and verbal communication skills. 
  • Must have access to and be able to operate a motor vehicle. 
  • Must have the ability to use a computer. 
  • Must be able to maintain confidentiality. 
  • Must possess a vehicle valid driver’s license and a willingness to travel as needed to client or organization locations throughout SE MN.

POSITION DESIGNATION:  Non-Exempt, Full Time

  • Paid Leave Time starts accruing at 5.08 hours per pay check (pro-rated for employees under Full-Time).
