Hands-on Care Needed For My Grandmother - Full-time / Part-time

My girlfriend and I are looking for one caregiver who will be able to cover a weekend shift Saturdays and Sundays from 8 a.m. to 8 p.m. (24 hours/week). We are typically home on the weekends to assist but we do our errands on one of the weekend days. She is non ambulatory and incontinent and requires brief checks and being rotated in bed every hour or so, we have an electric patient lift to get her out of bed as she does like to spend a few hours a day out in the living room or the backyard.
