Position Summary:

Our Registered Nurse, in Home Health and Hospice, is responsible for delivering skilled nursing care to patients in their homes, focusing on promoting comfort, dignity, and quality of life. The role involves assessing patients' needs, developing care plans, coordinating with other healthcare professionals, and providing compassionate support to patients and their families.


Position Type: Full Time/Permanent

Hourly Wage: $70 - $75 per/hour

Bonus: $10,000 Sign-On Bonus

Relocation Assistance: Up to $10,000

Housing Paid: Up to 6 months

Shift: Days


Expected Task:

  • Assessment and Care Planning: Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial needs. Develop individualized care plans in collaboration with the interdisciplinary team, incorporating patients' preferences and goals.


  • Skilled Nursing Care: Provide skilled nursing interventions such as wound care, medication management, infusion therapy, catheter care, and symptom management. Monitor patients' vital signs, observe for any changes in health status, and intervene promptly as needed.


  • Patient and Family Education: Educate patients and their families about their conditions, medications, and treatment plans. Empower them to manage their health at home effectively, recognize warning signs, and seek timely assistance when necessary.


  • Coordination of Care: Collaborate with physicians, therapists, social workers, and other members of the healthcare team to ensure seamless coordination of care. Communicate effectively to exchange relevant patient information and coordinate interventions to meet patients' needs.


  • Palliative and End-of-Life Care: Provide compassionate care and support to patients receiving palliative and end-of-life care at home. Manage symptoms, alleviate pain, and address emotional and spiritual needs to promote comfort and dignity in the final stages of life.


  • Documentation and Compliance: Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes according to regulatory standards and agency policies. Ensure compliance with state and federal regulations governing home health and hospice care.


  • Safety and Advocacy: Advocate for patients' rights and safety, promoting a safe environment conducive to healing and recovery. Identify and address potential risks or hazards in the home environment, collaborating with the interdisciplinary team to implement appropriate interventions.


Qualifications:

  1. Valid California RN license
  2. Minimum of one year of clinical nursing experience (MedSurg, Home Health or Hospice)
  3. BLS (Basic Life Support) certification
  4. Valid Driver's License
