The Home Health/Hospice RN provides skilled nursing care to patients in their homes by assessing the patient, identifying patient problems or potential problems, developing, implementing and evaluating nursing care plans and patient care. Works in team relationships with other home health and hospice providers.
Must be a graduate of an accredited school of nursing with an active Iowa license for Iowa residents or an active license in a compact state in which you reside. Two years of Home Health/Hospice nursing preferred.
