We are currently hiring for a position focusing on either case management or skills within the children department.

In 1964, we were established by a dedicated group of concerned citizens and government officials of Houston, Wabasha, and Winona counties. In 1965, we opened our first clinic in Winona.  Today we provide services in Winona, Houston, Wabasha, Goodhue and Fillmore counties.

TITLE:  Children’s Mental Health Practitioner 

 

DIVISION:  Children’s Community Based Services 

 

JOB SUMMARY: Coordinate and provide community services for children and youth with mental health conditions required to meet specific program eligibility requirements. Responsible for ensuring the individual receives services as outlined in a comprehensive evaluation. Responsible for educating and training individuals on basic skills and/or referring them to services and supports necessary to achieve service goals. 

 

JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS: 

  • Provide direct client care based on assigned case load and unique identified needs of each client.  
  • Teach clients basic skills and/or provide mental health case management in accordance with clinical, agency, and state guidelines and standards.  
  • Maintain case file documentation in compliance with state, agency, and program requirements.  
  • Maintain the amount of client contact and service level as required according to agency policy. 
  • Participate in required individual and/or group clinical supervision. 
  • Coordinate with other providers assigned to working with the individual in accordance with client and agency needs.  
  • Offer referrals and recommendations for community resources and services. 
  • Support clients during crisis situations.  
  • Provide transportation or arrange transportation for individuals as needed. 
  • Attend required organization and/or community meetings. 
  • Attend conferences, in-services, and training necessary to meet license, program, agency, and state requirements. 

 

PHYSICAL REQUIREMENTS FOR POSITION: Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs, and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office and community settings on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting up to 30 pounds. 

 

NON-ESSENTIAL FUNCTIONS:   Perform other duties as assigned by the Program Director or Program Manager. 

 

EQUIPMENT USED:   Computer, telephone, office equipment, vehicle. 

 

JOB QUALIFICATIONS AND REQUIREMENTS: 

  • Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes 
  • Worker Traits: Must possess a high level of human relation skills and an aptitude for effective decision making.  Must have good planning, organization, and communication skills (oral and written) and problem-solving skills. Independently motivated and responsible. 
  • Must be able to maintain confidentiality. 
  • Ability to work independently and as part of a team. 
  • Must possess a vehicle, valid driver’s license, and a willingness to travel as needed to client or organization locations throughout SE MN.

Reports To: Children’s Community Based Program Director or Program Manager depending on location and program assigned.

  • Paid Leave Time starts accruing at 5.08 hours per pay check (pro-rated for employees under Full-Time).
