Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The Children and Families Lead Care Manager is responsible for coordinating the care of children and families that are residing in Illumination Foundation’s family emergency shelters. LCMs will conduct Comprehensive Health Assessments, complete developmental screenings, and provide referrals and linkages to resources. LCMs will provide follow-up support in the form of Care Plan to families after leaving the program to ensure children participate in ongoing services.

The pay range for this position is $23.00 - $26.00 per hour, depending on experience.

Responsibilities

Administrative/ Record Keeping
  • Maintain updated & accurate case notes within 24 – 48 hours of services provided, records, & relevant paperwork on Illumination Foundation’s internal EHR system: AICA & HMIS
  • Provide a minimum of 2 hours of services every 25 days to a caseload of 30 clients.
  • Complete ECM Enrollment protocols to track client’s consent to enroll in the program.
  • Input and enter all data from screenings (ASQ, ASQ-SE, SDQ, PEARLS etc.) on relevant documents, AICA, and HMIS. 
  • Utilize the Children’s Registry for entry of Developmental Screenings data and referrals. 
  • Diligently Track program services in the form of an on-going Care Plan, for purposes of data collection, and proof of desired outcomes.
  • Maintain a weekly log and update case files/notes as necessary.
  • Collaborate with other departments at Illumination Foundation as necessary.
  • Complete referral paperwork as deemed appropriate for children and families.
  • Collaborate with external partner agencies and organizations.
Direct Services
  • Complete a Health Risk Assessment (HRA) after enrollment into ECM.
  • Conduct developmental screenings for all children who enter ECM (ASQ, ASQ-SE, SDQ, PEARLS etc.).
  • Create a Care Plan based on the health & social needs found during the HRA.
  • Attend scheduled appointments, as deemed necessary, and advocate for clients at scheduled appointments relating to client’s housing, health, education, and social and emotional wellbeing.
  • Regularly meet with Clinical Consultant to review client health/medical needs to provide advocacy, health and preventative care education
  • Educate parents on topics around parenting, child development and available services.
  • Address familial concerns as they arise with parents.
  • Provide transportation for client appointments if deemed necessary.
  • Provide referrals and track linkages to relevant services for children on caseload.
  • Attend team meetings, case conferences and supervision as scheduled.
  • Collaborate with the Children's Department to provide children’s services to families staying in IF emergency and bridge housing facilities.
  • Continually update resources for activities and lesson plans.
  • Monitor for signs of emotional or developmental troubles in children and communicate to the supervisor.
  • As a mandated reporter, monitor for signs of emotional, physical, and sexual abuse and communicate to CPS as deemed necessary by law.

Preferred Experience/ Minimum Qualifications

Required:

  • 1-2 years relevant experience.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Valid CA Driver’s License and eligibility for company vehicle insurance. 

Preferred:

  • Bachelor’s degree in human services, child development or other related field
  • Bilingual in English and Spanish
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams)
