Provides services at the OCO Crisis & Development Office, in the community, program sites, partners, schools, consumer homes,
and other service providers as assigned.

Consumers:
• Engages and provides advocacy, linkage, navigation & retention into services for the target population(s) including those individuals
seeking workforce and educational supports
• Is responsible for conducting Evidence Based Programs (EBPs) as designated by funder in community settings with target
population(s) reaching up to 50 individuals annually with multi session EBPs
• Is responsible for the case files of up to 50 individuals & families.
• Responds to consumers who are contacting in person, through a variety of multimedia avenues and on the phone to provide
information about services & the status of their services,
• Works as part of a team to staff community hub sessions, provide home visits, assessments, and education to increase self-sufficiency,
provide services, support and follow-up.
• Independently assesses the additional work, health, & education needs of individuals and families.
• Advocates, accompanies, assists participants with scheduling appointments, self-advocacy; and/or negotiates on behalf of
consumers.
• Assists with job readiness skills, job placement, and interview preparedness
• Provide trainings, monitoring and ongoing support to volunteers, staff, and community partners
• Manages services for consumers and facilitates case management meetings with partners and other providers.
• Works cooperatively with schools, community members, agencies and other service providers.
Responsible for:
• Acts as a role model for peers & partners in helping people living in poverty; demonstrates cultural competencies for effectively working
with individuals/families to help navigate systems and reach goals of increase self-sustainability including: meeting & serving people
where they are at in various locations throughout the community including homes, partner locations, schools and other locations where
the target population(s) congregate. Promotes strength based & non-judgmental services for all consumers.
• Organize and facilitate partnership meetings to advance the objectives of the project
• Collects all funder required session, cycle, and consumer information.
• Tracking and compiling statistics related to program activities and outcomes
• Completing reports for various project stakeholders
• Providing case management services to enrolled individuals and their collaterals
• Staying up to date with best practices in the field and sharing information with project partners and co-workers

Other:
• Develops and maintains effective working relationships with community partners, such as schools, employers, and other service
providers
• Works cooperatively with volunteer staff and utilizes a core of volunteers and interns appropriately to assist in providing services &
supports to increase self-sufficiency.
• Carries out other special projects as assigned.

Full Time - 37.5 hours per week

Grade 8

Monday - 8:30am - 4:30pm

Tuesday - 8:30am - 4:30pm

Wednesday - 8:30am - 4:30pm

Thursday - 10:30am - 6:30pm

Friday - 8:30am - 4:30pm
