Responsibilities The hospice aide/home health aide provides personal care, homemaking services and other supportive services for people in their place of residence. All such services will be provided in accordance with an individualized, comprehensive patient care plan and under the supervision of a Registered Nurse and/or a Physical, Occupational, or Speech Therapist, when appropriate, in compliance with the agency’s policies and standards of practice. Qualifications 18 years or older and completion of a state approved license program for Home Health Aide or Nursing Assistant training program required. Previous work experience as a certified nursing assistant within the past 12 months is preferred. Experience with end-of-life or geriatric populations preferred. Must be able to work independently. Must foster team atmosphere by maintaining open communication with interdisciplinary team; must have the ability to recognize problems/concerns that should be brought to the attention of the proper personnel. Able and willing to drive-in all-weather conditions and cover service area. Must provide personal mode of transportation to patient’s place of residence. Valid driver’s license, proof of auto insurance, and acceptable driving record as defined by policy required. Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. If a candidate does not currently have their Basic Life Support for Health Care Providers, certification will be included as part of the orientation program. This position sees patients in the Southeast MN geography including the town of Albert Lea and surrounding areas. Mileage is reimbursed.
