Salary:

JOB PURPOSE: The Supportive Counselor / Case Manager coordinates and monitors the assessment, treatment planning process and follow-up for each client, and serves as an advocate and liaison for and on behalf of clients’ rights and benefits.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Ensures that all participants complete the individual plan for contracted mental health services.
  • Facilitates community resource availability based on client needs (e.g., day program, guardians) to ensure maximum social integration.
  • Evaluates, facilitates and monitors services and clients’ progress in relationship to established goals and objectives. Documents monthly in the medical record.
  • Provides monthly monitoring overview and ensures that all services have been delivered and properly accounted for before submitting billings to fiscal office.
  • Facilitates client benefits (e.g., SSI, Medicaid, SSB, Medicare).
  • Reviews and coordinates with the team and provider on decisions involving client. Implements plan of action.
  • Provides crisis intervention for assigned caseload.
  • Maintains assigned medical records as required by medical record procedures.
  • Provide supportive group and individual counseling services.
  • Provides case management services when appropriate.
  • Other duties may be assigned.


QUALIFICATIONS/EDUCATION/EXPERIENCE:

  • Bachelor's degree in human services field.


COMPETENCIES:

  • Decision Making.
  • Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures and governmental regulations. Ability to write reports, business correspondence and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.
  • Ability to calculate figures and amounts such as percentages, mean, mode and median.
  • Ability to draw and interpret bar graphs.
  • Ability to define problems, collect data, establish facts and draw valid conclusions.


PHYSICAL DEMANDS:

  • Ability to lift minimum 25 pounds.
  • Ability to perform job responsibilities in a facility with multiple levels and without elevator or mechanical transportation.
  • Operates a computer and keyboard.

This job operates in an outpatient facility environment.
