Millennium Home Health is looking for compassionate and reliable Physical Therapists and Physical Therapist Assistants/Aides who want to build a rewarding career in mobile healthcare. Full time and part time schedules are available. The position is per diem and you will be paid  per visit, not per hour. If you are interested in full time employment, that can be discussed as well.

You will be responsible for providing direct patient care. Your responsibilities will include evaluating the patient’s functional status and physical therapy needs, developing an appropriate plan of care, following the plan of care, and working collaboratively with members of the team to help meet positive patient care outcomes.

 

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:

  • Evaluates the patient's functional status and physical therapy needs and develops an appropriate plan of care.
  • Treats patients to relieve pain, improve mobility, restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
  • Visits patients according to Plan of Treatment, completes progress notes timely.
  • Completes all patient evaluations and develops the PT plan of care within state-specific guidelines.
  • Reports outcomes of evaluation, goals, and anticipated projected frequency of care after the evaluation.
  • Develops appropriate home programs to maintain and enhance the performance of the patient in his/her own environment.
  • Participates in transition/discharge planning.
  • Provides/recommends/fabricates adaptive devices, orthotics, or prosthetics.
  • Trains patient, family, and appropriate staff in the use or application of the above.
  • Provides services within the scope of practice as defined by the state board of physical therapy, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Maintains patient confidence and protects facility operations by keeping information confidential.
  • Supervise the activities of physical therapy assistants when required.
  • Maintains a safe and clean working environment by protecting patients and employees by adhering to infection-control policies and protocols.

QUALIFICATIONS:

  • Must be currently licensed to practice as a Physical Therapist and compliant with accepted professional standards and practices.
  • Possesses and maintains current CPR certification.
  • Complies with accepted professional standards and practice.
  • Demonstrates excellent verbal and written communication, and organization skills.
  • Results-oriented with strong critical thinking and problem-solving skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  • Be comfortable driving a distance of approximately 30-50 miles from home.
