You are the Primary Care Provider with your own panel of patients:

  • Weekday opportunity in Outpatient Primary Care Clinic
  • This is full-time position (4 days clinic, 1 day admin)
  • Collaborate with a team of providers from all specialties, plus integrated behavioral health clinicians, community health advocates, and nurse case managers.
  • Lead your team – a dedicated RN and Medical Assistant – to provide quality care to a diverse multi-cultural underserved patient population.

About our Clinics: Providence Community Health Centers (PCHC) is the largest Federally Qualified Health Center in Rhode Island with nine neighborhood-based clinics - all Patient- Centered Medical Homes. Our in-house services, on same EHR, include optometry, podiatry, dental, dermatology, nephrology, rheumatology and asthma/allergy.

Ideal candidates will be: Family Nurse Practitioner who is confident caring for all ages; minimum 3 years of primary care experience; ability to work independently as well as collaborate as part of a larger provider group; confident learning new EHR; Licensed/eligible in Rhode Island; Basic Life Support (BLS) certification; Multi-cultural experience highly valued.

Overview:

Work with other personnel of the health center and the physicians, to make the delivery of health care more efficient, more comprehensive, and improve the quality in accordance with the policies and procedures of PCHC, the laws of Rhode Island, and appropriate standards of good care. All duties/responsibilities are performed across life cycle (infants, children, adolescents, adults, and geriatric) as appropriate for training and certification.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Take a complete medical history and do a physical examination on PCHC patients as clinically appropriate.
  2. Diagnose and treat common medical problems in your specific fields of competence. Consult with the appropriate PCHC physician at the FNP’s health center, or with the appropriate PCHC physician on call, or with the Associate Medical Director, as needed and in accordance with PCHC policies and procedures to assure good care.
  3. Manage acute and chronic illness collaborating as needed with the PCHC M.D. and assist in coordinating care for these patients with other members of the health center and/or with services outside of the health center.
  4. Provide patient education and preventive medicine as clinically indicated.
  5. Provide counseling and support as clinically appropriate, in areas of social problems recognizing the emotional component of illness, making referrals to mental health professionals as needed.

QUALIFICATIONS/EDUCATION/EXPERIENCE:

  1. Education – RN and Family Nurse Practitioner training program in accredited college or university.
  2. Must be licensed as RN and as a Nurse Practitioner with prescriptive privileges in Rhode Island.
  3. Must be certified as a Family Nurse Practitioner.
  4. Must conform to prescribed standards for continuing education as set down by appropriate state or national advisory boards.
  5. Experience – at least one year in a community health setting.
