Overview

VNS Health Hospice RNs provide holistic end-of-life care to New Yorkers so they can be cared for in their final moments surrounded by family and friends, in the familiar comfort of home. Our hospice team is known as VNS Health’s most patient nurses who educate family members on topics like disease progression, wound care, and medication, while delivering personalized nursing plans. Hospice nurses provide comprehensive clinical care as part of an interdisciplinary team that includes dieticians, spiritual care counselors, and bereavement specialists. Compensation:

$104,868.00 - $130,268.00 Annual

Our clinical compensation packages start with highly competitive base pay and build from there to recognize your education, clinical experience, certifications, and ongoing commitment to providing care to patients and members in high need areas.

What You Will Do

  • Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care
  • Initiate quality-driven care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently
  • Practice independently in the community, traveling to patients’ homes or care facilities, using effective time management and agility to manage weekly caseload and other duties
  • Think critically to proactively organize, plan, and obtain supplies and equipment needed for patient care
  • Complete individualized comprehensive assessments and related documentation, including but not limited to social determinants of health, OASIS assessments, wound care, and reporting vital sign changes    
  • Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community 
  • Provide empowering education and support to patients and their families to help them understand and navigate end of life treatment 

Qualifications 

  • Current license to practice as a Registered Nurse in New York State  
  • Minimum of one year nursing experience in a medical, surgical, or hospice environment
  • Valid driver's license or NYS Non-Driver photo ID card may be required  
  • Strong clinical assessment and critical thinking skills 
  • Excellent communication and interpersonal abilities 
  • Ability to work independently and as part of a team 
  • Must be able to walk up and down steps to access patient residences and transport required supplies weighing approximately 25 – 30 lbs.
