Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine hospice care needs.
  • Develop, initiate, and revise care plans based on patient needs and status.
  • Administer medications and treatments as prescribed by physicians.
  • Offer counseling and support to patients and families, including education on self-care techniques.
  • Coordinate patient care and communication with interdisciplinary teams.
  • Participate in on-call duties and ensure necessary equipment and services are available.

Key Qualifications:

  • Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
  • Valid driver's license, personal automobile, and insurance coverage.
  • One year of hospice care experience is preferred.
  • Case Management experience and supervision of hospice aides is a plus.
  • Excellent observation, communication, problem-solving, and nursing skills.
  • Ability to perform physical tasks such as lifting, positioning, and transferring patients.
  • Effective social interaction, communication, and organizational skills.
