Salary:

Family Practice Center, P.C. is currently seeking a Part-Time (20 hours per week) Community Health worker in our Administration office in Selinsgrove.    This position will serve as a liaison between the patient, health services, and our community to facilitate access to quality services and health care.  Home support staff will be making visits to patients in their homes.  In person as well as computer and telephonic communication with patients, providers and staff.  Duties will vary based on patient needs.

Duties/Responsibilities:

  • Primary liaison between patient, social worker, discharge planning and chronic care management team, and providers.
  • Organize and review medications in the home.
  • Helping patients complete applications and other paperwork.
  • Helping people understand their health condition(s) and develop strategies to improve their health and well being.
  • Delivering health information using culturally appropriate terms and concepts.
  • Linking people to health care/social service resources.
  • Provide in person support in developing the skills necessary for integrating lifestyle changes into daily life.
  • Reports potential risks for the patient to the healthcare team.
  • Documentation in patients’ medical records after all interactions.
  • Help patients get appropriate screenings and referrals for health and social services.
  • Work alongside social worker/TOC/CCM nurses to track an individual’s progress toward meeting health goals.
  • Reinforces prescribed medication regimes and treatments in accordance with the provider orders.
  • Observes, records and reports patients’ conditions, family member interactions/ availability, and living environment.
  • Works under the direction of Family Practice Center providers.

Education and Experience:

  • Medical Assistant, Certified Nursing Assistant preferred.
  • High School diploma or equivalent required.
  • Previous experience preferred.


Physical Requirements:

  • Sitting up to 4-8 hours per day.
  • Walking up to 1-3 hours per day.
  • Standing up to 1-3 hours per day.
  • Lifting/pushing/pulling up to 30lbs.
