YOUTH HOMELESSNESS DEMONSTRATION PROJECT (YHDP) HOUSING NAVIGATOR

Reports to: Housing Navigation Program Manager 

 
Job Summary: The Youth Homelessness Demonstration Project (YHDP) Housing Navigator provides direct program services, program implementation, and staff training to FosterAdopt Connect’s, YHDP Housing Navigators. This program provides trauma-informed community-based services, case management, and housing support to youth experiencing housing instability and homelessness up to their 24 birthday. Housing Navigators assist clients in accessing stable and safe long term housing, locating and identifying resources for youth, and coordinating supportive services. 

 

Responsibilities: Primary responsibilities include but are not limited to: 

 

Administrative Leadership and Management  

  • Follow all program policies and procedures as required to provide solution-focused housing interventions to expedite housing placement for clients.  
  • Responsible for creating and maintaining community partnerships to assist in the service delivery identified as needed for clients’ success.  
  • Provide quality and hands-on case management to youth; maintain flexibility in scheduling appointment times. 
  • Practice a Housing First philosophy with clients. 
  • Maintain clients file and records within SAM database and other required databases. 
  • Complete month-end reports as required. 
  • Deliver responsible resources to youth through community stakeholders and internally within FAC. 
  • Responsible for identifying goals and action steps in order for clients to obtain appropriate housing. 
  • Travel required, with or without accommodation, to attend and/or present in-service training or presentations, Balance of State required meetings, and direct care to clients face to face at a minimum monthly. 
  • Provide transportation when needed to clients lacking the ability to locate reliable transit themselves. 
  • Responsible for making community connections to increase program referrals as assigned (distribute program brochures, schedule informational meetings, speak at community presentations, booth exhibits, etc.).  
  • Participate in ongoing meetings/professional memberships and trainings as required by the Program Manager and/or Senior Director of Youth Housing. 
  • Participate as an active team member with colleagues of all demographics in order to effectively and efficiently serve clients.  
  • Maintain strong, open communication with direct supervisor regarding services delivered, concerns, or questions. 

 

Qualifications & Preferred Skills 

  • Bachelor’s Degree (preferred) in social work, psychology, public administration, or a related human services field and three (1+) years of experience working in non-profit social service agencies with three (1+) years of management experience.    
  • Previous experience working with at-risk teen population required. Lived experience with housing instability preferred. 
  • Demonstrated resourcefulness in setting priorities, proposing new ways of creating efficiencies, and guiding investment in people and systems.  
  • Solid commitment to the principles of support and advocacy for youth. 
  • The ability to collaborate and advocate for positive outcomes, using excellent interpersonal skills in multi-disciplinary, diverse, and dynamic professional teams. 
  • Strong verbal communication skills and demonstrated ability to write clearly and persuasively.  
  • Must have ability to quickly establish rapport and build trust.  
  • Flexible and a self-starter; able to multi-task while also being highly detail-oriented while meeting all deadlines.  
  • Work as a necessary part of a team effort with colleagues of all demographics.  
  • Model ethical and professional behavior at all times. 
  • Valid Missouri Driver’s License and car insurance. 
  • Ability to be flexible in work hours to meet client needs.
