Clinical Social Worker - Long Term Care

The Adrian Dominican Sisters are seeking a Full Time Clinical Social Worker to join our care team at our Motherhouse campus in Adrian, MI.

In this role you will: Assist in the successful transition of Sisters as they enter the Dominican Life Center, our continuum of care facility, and also when they transition between areas of residence and provide support to enable each Sister to function at the highest possible level of social and emotional wellness.

Pay and Hours: $63,000-$73,000/year based on experience. Monday - Friday 8am - 4:30pm.

What you will be responsible for:

Clinical Social Work Role:

  • Coordinate care and aid transition of residents coming to, discharged from, or moved within the Dominican Life Center (DLC) on a temporary/permanent basis.
  • Work with Chapter/Mission Prioress regarding Sisters coming to or being discharged from the DLC.
  • Work in close collaboration with physicians and caregivers in resident evaluation and treatment to further their understanding of social and emotional factors underlying health problems.
  • Obtains a psycho, social, interest-based assessment for each resident.
  • Provides counseling and/or depression screening as requested.
  • Provides therapeutic intervention to assist resident in coping with transition and adjustment to the DLC.
  • Participates in the development of a written, interdisciplinary care plan.
  • Documents according to Sisters' Plans of Care information for other Caregivers and Chapter/Mission Prioress.
  • Provides educational programs and groups appropriate to the needs of the residents.

Interdisciplinary Role:

  • Schedules and actively participates in New Admission and Care Conferences.
  • Participates in other DLC committees as appropriate.
  • Works closely with the Mental Health Provider.
  • Develops, schedules, and facilitates resident support groups.
  • Provide grief and loss counseling support as Sisters deal with end of life concerns; collaborate with chaplains as needed. Collaborates with hospice care team in identifying end of life issues and aid Sister to address issues.
  • Responds appropriately and complies with all safety policies and procedures to safety hazards and/or emergencies following written campus procedures.

REQUIREMENTS:

Education: MSW or equivalent work experience required and current license to practice Social Work.

Experience: At least 3-5 years of experience in social work in a long-term health care environment required.

Knowledge, Skills and Abilities: 3-5 years experience required of the following:

  • Recent clinical experience pertinent to issues of death, loss, and grief.
  • Interest/experience in gerontology, emotionally and spiritually mature, communicate effectively with tact and professionalism, commitment and sensitivity to Sisters dealing with life issues.
  • Able to function under stress and to cope with crisis situations and to multi-task and be flexible with duties and schedule.

Physical: Must be able to sit for long periods of time. Be able to move from location to location on campus. Must be sighted and capable of hearing. Annual influenza vaccination is required for this position.
