We are seeking a full-time RN Case Manager who would like to join our hospice team!
Job Description:
- RN Case Manager, M-F, Dayshift
- Create your own visit schedule and have work/life balance.
- As a full-time RN case manager, you will manage a caseload of 10-15 patients.
- As a hospice nurse, you will provide patients and families education and support throughout the end-of-life process and address patient's physical, emotional and spiritual needs.
- You will work closely with the hospice interdisciplinary team to create and implement a holistic plan of care congruent with the patient's and family's wishes, focusing on comfort, support and quality of life.
- Pay: Starting at $40/hour ($83,200 annually), with guaranteed minimum of 40 hours paid per week, even if you work less.

- Ability to work autonomously
- Timely charting in EHR
- Positive and friendly demeanor
- Ability to build rapport with patients
- Ability to handle physical workload
- Excellent written and verbal communication skills
- Strong organizational and time management skills
- High level of emotional intelligence
- Reliable transportation
- Valid driver's license and vehicle insurance
- Valid New Mexico RN license
- BLS certification
- Preferred: 1 year critical care, skilled nursing, home health or hospice experience
