What You'll Do:

• Play an instrumental role in our program, aiming to deliver medical and social support to our patients where they need it most, in their homes.

• Creating Care plans for chronically ill frail elderly seniors.

• Educate patients on the importance of consistent health monitoring, medication compliance, and chronic condition management.

• Work collaboratively, participating in weekly interdisciplinary team meetings to craft tailored care plans, bridging potential gaps in care.

• Remain updated with current clinical standards of care, showcasing your ability to operate independently and prioritize patient needs.
