Nurse Practitioner or PA or Physician

We perform independent physical exams (consultative exams) for Illinois disability. Each exam last up to 20 minutes.

$50 per claimant seen; work as independent contractor. Make up to $1200/day.

Chief complaint/Brief history.

Review of systems/Vital signs.

Basic physical: Mostly range of motion/strength; description of gait; use of hands; etc.

Occasional spirometry testing (0-3 usually) - additional $20/test (takes about 10-12 minutes).

We DO NOT make the decision on who gets disability - we only provide a report as part of the process.

Full Illinois license required - Professional liability insurance provided free of charge.

Usually 20-25 slots scheduled; 20 minutes each; 8 AM - 5:00 PM (typically - this is flexible); any day of the week INCLUDING Sundays.

Work as little or as much as you want - usually 1 exam day scheduled per week.
